The enrollment fee is per location and will be automatically assessed monthly.  The monthly fee includes:

$ 85.00 - which PPCN is collecting for CPESN USA and remitting to them

$14.00 - which PPCN is collecting and retaining to help offset PPCN administrative and operative costs.

Value Drug Company Primary Pharmacies:  Value Drug is providing a subsidy for your enrollment fee of $45 ($40 for CPESN and $5 for PPCN)  DO NOT USE THIS ENROLLMENT FEE for any pharmacy locations that are Value primary. Please use this instead:   Value Early enrollment

You can pay for multiple locations with one charge by changing the quantity or if you want them separately charged - please enter separately for each location.

If you enrolled 6 locations under your corporate entity but want to pay for 3 with one credit card and 3 with another - be sure to enter each and fill in the specifics for which location in the above box.
